Conference Application

Planning on attending the 2012 Conference? Eveyone attending needs to fill out the spreadsheet to attend. This is how we keep track of who is coming, their meals etc. 

The spreadsheet is available on the website or by emailing LeansToFar@aol.com. Payment of fees can be made after filling the form out and sending to jim greenwald. If you are paying by check mail to; MWSA, P.O. Box 264, Bridgeville, PA 15017 or if you wish to be invoiced and pay through paypal, make your selection of payment method clear on the spreadsheet.

Be certain to select your banquet meal, list any guest and their meal. If placing books on the book table write in the title(s) and the selling price(s) in round numbers, no odd change.
